Regions
Full details from South Yorkshire's Community Foundation (funded by NHS South Yorkshire ICB)
South Yorkshire's Community Foundation runs the Prevention Accelerator for NHS South Yorkshire ICB for 2026 to 2028. It funds groups in Doncaster and Sheffield to support people with, or at risk of, cardiovascular conditions, kidney disease and diabetes, in groups where these are underdiagnosed: the most deprived communities, Global Majority communities, and men aged 40 to 74. Applications focusing on Central Doncaster and Hyde Park, Burngreave and Grimesthorpe, and Woodthorpe are viewed favourably. Grants are up to £10,000 for single organisations, or up to £20,000 for partnerships or cross-regional projects; larger proposals can be discussed. Work can include physical activity, creative health and green or blue social prescribing, with delivery costs and a proportionate contribution to overheads. The programme aims to fund 20 to 25 projects split evenly between the two places. The first round closes at 5pm on 23 November 2026. Activity starts no earlier than 15 February 2027 and must finish by 31 March 2028, with bi-monthly monitoring.
Read the Prevention Accelerator guidance notes and example monitoring form on South Yorkshire's Community Foundation's fund page, check SYCF's due diligence guidelines, then apply through SYCF's online application before 5pm on 23 November 2026.
